If you post photos of anyone underage, I strongly recommend that you review the activity on your Flickr account on a regular basis. To do so, go to the bottom of the page, and click on "Around You" in the Activity row.

When you add a photo to a group, any member of that group can add notes, tags, and comments without your permission. Malicious users will sometimes add adult notes and tags to other person's photos.

Checking your account activity will allow you to easily keep track of this. The only way to prevent someone from doing that is to block that individual person.

If someone faves your child's photo, also review all their faves by clicking the pink star. Sometimes you will find that the person's other faves are primarily adult images. Also check their photostream for what flickr would describe as being "creepy." Again, a block is in order.

I blocked 2 people this morning for those reasons, so I recommend that everyone stay alert for this problem.
